Job Title/Position: Medical Social Worker
Reports to: Clinical Supervisor

Position Overview: The medical social worker plays a vital role in delivering high-quality social work services in a hospice setting. This position is focused on implementing established standards of care and providing comprehensive support to patients and their families.

KEY RESPONSIBILITIES:

Patient Support:
  1. Evaluates the psychosocial conditions of patients in relation to their health status and surroundings, and communicates insights to the nursing staff.
  2. Conducts social assessments and formulates intervention strategies based on assessment results.
  3. Keeps accurate clinical documentation for all patients referred for social work services.
  4. Offers information and referral assistance to patients and their families regarding practical and environmental concerns.
  5. Acts as a resource for patients, families, and community organizations.
  6. Facilitates connections between patients, families, and community resources.
  7. Fosters collaborative relationships with healthcare team members to enhance patient care.
  8. Develops and maintains partnerships with public and private organizations to support patient needs.
  9. Engages in ongoing education and training programs.
  10. Contributes to the overall care plan and participates in case discussions as necessary.
  11. Assists medical professionals in recognizing significant social and emotional factors impacting health issues.
  12. Involved in planning for patient discharge.
  13. Performs additional duties as assigned by the Clinical Director/Supervisor.
  14. The responsibilities outlined above are intended to provide a general overview of the role and may include other related tasks as required.
QUALIFICATIONS:

1. Bachelor's degree in social work from an accredited program recognized by the Council on Social Work Education.

2. Current licensure as a clinical social worker by the Board of Behavioral Sciences.

3. At least one year of experience in a healthcare environment.

4. Experience in home health care is preferred.

5. Must possess and maintain a valid CPR certification.

6. Must have a valid driver's license and access to a reliable, insured vehicle.

SKILLS REQUIRED:

1. Strong verbal and written communication skills, along with excellent organizational abilities.

2. Ability to lift, push, or pull weights of 15-20 pounds regularly, and occasionally manage heavier weights.

3. Capable of performing various physical activities such as sitting, standing, walking, and driving as required during work hours.

4. Ability to manage the emotional and psychological demands of the role.
