Position Overview: The medical social worker plays a crucial role in delivering high-quality social work services in a hospice setting. This position is vital for ensuring that patients receive comprehensive care that addresses their emotional and social needs.
KEY RESPONSIBILITIES:
Patient Support:
- Evaluates the psychosocial condition of patients in relation to their health issues and surroundings, and shares insights with the nursing staff.
- Conducts social assessments and formulates intervention strategies based on the assessment results.
- Keeps detailed clinical records for all patients referred for social work assistance.
- Offers information and referral services to patients and their families regarding practical and environmental concerns.
- Provides guidance to patients, families, and community organizations.
- Acts as a bridge between patients, families, and community resources.
- Fosters collaborative relationships with other healthcare professionals to enhance patient care.
- Develops and maintains partnerships with public and private organizations to support patient needs.
- Engages in ongoing education and training programs.
- Contributes to the creation of comprehensive care plans and participates in case discussions as necessary.
- Assists medical professionals in recognizing significant social and emotional factors that may impact health issues.
- Involved in planning for patient discharge.
- Performs additional responsibilities as assigned by the Clinical Director/Supervisor.
- The above duties represent a summary of the primary functions and responsibilities of this role. Employees may be asked to undertake other job-related tasks as needed.
1. Bachelor's degree in social work from a program accredited by the Council on Social Work Education.
2. Licensed as a clinical social worker by the relevant state board.
3. At least one year of experience in a healthcare environment.
4. Experience in home health care is preferred.
5. Current CPR certification is required.
6. Must possess a valid driver's license and have access to a reliable vehicle that meets state or organizational insurance requirements.
SKILLS REQUIRED:
1. Strong verbal and written communication skills, along with excellent organizational abilities.
2. Capable of lifting and moving 15-20 pounds regularly, with the ability to manage heavier weights as needed.
3. Ability to perform various physical activities, including sitting, standing, walking, and driving to patient locations.
4. Must be able to handle the emotional and mental demands of the role.
