**Salary**: $93,000 - $118,000 per year (dependent on location)

Job Description
As a Hospice Care Coordinator RN, you will provide assessment and evaluation support to ensure that our patients receive the best possible care. You will work closely with medical staff, administrators, and other healthcare professionals to develop and implement quality improvement projects, conduct root cause analyses, and facilitate multidisciplinary teams to drive performance improvement. Your expertise will be invaluable in helping us maintain our commitment to excellence in patient care.

**Required Skills and Qualifications**:
  • Bachelor's degree in Nursing or related field
  • Minimum 3 years of experience in nursing, preferably in a hospice or palliative care setting
  • Current licensure as a Registered Nurse in the state of hire
