Job Summary

The Registered Nurse Case Manager plays a vital role in providing comprehensive care to patients and their families in the comfort of their own homes. As a key member of the Luminary Hospice team, this professional will be responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating with healthcare providers to ensure seamless care delivery.

Key Responsibilities
  • Conduct thorough assessments of patients and their families to identify care needs and develop personalized care plans.
  • Collaborate with healthcare providers to ensure timely and effective care delivery, including medication management and symptom control.
  • Provide education and support to patients and their families on disease management, symptom control, and end-of-life care.
  • Coordinate with community resources and healthcare providers to ensure patients receive comprehensive care.
  • Document patient progress and care plans in a timely and accurate manner.
  • Participate in interdisciplinary team meetings to discuss patient care and develop care plans.
Requirements and Qualifications
  • Bachelor's or Associate's degree in nursing or related field.
  • Active nursing license in good standing.
  • At least 2-5 years of experience in hospice or palliative care.
  • Knowledge of hospice care and services provided to patients and families.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
