Registered Nurse Case Manager

The Registered Nurse Case Manager plays a vital role in providing comprehensive care to patients and their families in the comfort of their own homes. As a key member of the Luminary Hospice team, this professional will be responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating with healthcare providers to ensure seamless care delivery.
Key Responsibilities- Conduct thorough assessments of patients and their families to identify care needs and develop personalized care plans.
- Collaborate with healthcare providers to ensure timely and effective care delivery, including medication management and symptom control.
- Provide education and support to patients and their families on disease management, treatment options, and end-of-life care.
- Coordinate with community resources and healthcare services to ensure patients receive comprehensive care.
- Document patient progress and care plans in a timely and accurate manner.
- Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving patient outcomes.
- Bachelor's or Associate's degree in nursing or related field.
- Active nursing license in good standing.
- At least 2-5 years of experience in hospice or palliative care, with a focus on case management and patient education.
- Knowledge of Medicare Hospice Certification and CHAP guidelines.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
- Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
The Registered Nurse Case Manager will work in a fast-paced, dynamic environment with a focus on providing high-quality patient care. The ideal candidate will be able to work independently and as part of a team, with a strong commitment to delivering exceptional patient outcomes.
Physical DemandsThe Registered Nurse Case Manager will be required to sit and use hands to finger, handle, or feel for extended periods. The employee must occasionally lift and/or move up to 50 pounds.
