Responsibilities:

  • Conduct comprehensive assessments of patients and families to determine their unique needs and develop individualized care plans.
  • Provide expert nursing care, including pain and symptom management, to ensure optimal comfort and quality of life for patients.
  • Collaborate with the interdisciplinary team to coordinate care and services, including medication management, wound care, and bereavement support.
  • Act as a liaison between patients, families, and healthcare providers to ensure seamless communication and care coordination.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
  • Stay up-to-date on industry developments, best practices, and regulatory requirements to ensure compliance and excellence in care delivery.

Requirements:

  • Bachelor's or Associate's degree in nursing or related field.
  • Active nursing license in good standing.
  • At least 2-5 years of experience in hospice or palliative care.
  • Knowledge of Medicare Hospice Certification and CHAP guidelines.
  • Excellent communication, interpersonal, and problem-solving skills.
