Outpatient Clinical Case Manager

Arms Acres Inc. is seeking a dedicated Clinical Case Manager to enhance our outpatient services. This position involves delivering thorough clinical case management support, which encompasses: assessments, individual and group therapy, family interventions, treatment plan coordination, and collaboration with client support systems, including social services and managed care.
Available Shifts: Sunday - Thursday or Tuesday - Saturday, specific hours to be discussed.
Key Responsibilities:- Conducts comprehensive evaluations of patient functionality in accordance with facility protocols.
- Facilitates the creation and execution of multidisciplinary treatment plans; oversees treatment plan reviews within designated timelines.
- Records patient progress and interventions in case files, adhering to regulatory time constraints.
- Delivers individual, group, and family therapy (including psycho-education and life skills training) as specified in treatment plans.
- Engages in utilization reviews, provides progress documentation, and formulates aftercare strategies in collaboration with referring agencies and managed care entities.
- Participates in program and quality enhancement initiatives within the facility.
- Performs additional responsibilities as assigned.
Essential Qualifications:
- Certification as CASAC-T, CASAC, MSW, LMSW, LCSW, or LMHC is mandatory.
- A Master’s Degree in a relevant field is required.
- Experience in outpatient substance abuse counseling is preferred.
- Bilingual candidates are preferred.
