Job Summary

We are seeking an experienced Advanced Practice Registered Nurse to join our team at InterCommunity. As a key member of our healthcare team, you will provide high-quality primary care services to children and adolescents in our School-Based Health Centers.

Key Responsibilities
  • Provide comprehensive primary care services, including well-child visits, sick visits, and urgent care, to children and adolescents in our School-Based Health Centers.
  • Conduct thorough health assessments, develop treatment plans, and provide education to patients and families on healthy habits and disease prevention.
  • Collaborate with school nurses, administrators, and other healthcare professionals to ensure seamless care coordination and communication.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and outcomes.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices in pediatric primary care.
Requirements
  • Master's degree in Nursing and two (2) plus years related experience.
  • Current license as an Advanced Practice Registered Nurse issued by the Connecticut Department of Public Health.
  • Nurse Practitioner (PNP) or Family Nurse Practitioner (FNP) certification preferred.
