Job Title: Lead Case Manager

The United Planning Organization (UPO) is seeking a highly skilled and compassionate Lead Case Manager to join our team. As a key member of our Community Health division, you will play a vital role in providing exceptional support services to individuals and families in need.

Job Summary:

The Lead Case Manager will assist the Program Director in administrative responsibilities and carry out assigned case management duties. This includes client assessment and planning, resource coordination, advocacy, crisis intervention, documentation, staff training, collaboration with community partners, and ensuring quality service delivery.

Key Responsibilities:
  • Conduct thorough assessments of clients' needs, strengths, and goals, and develop individualized service plans in collaboration with clients.
  • Identify and coordinate access to housing resources, social services, mental health support, substance abuse treatment, and other community resources to meet clients' needs and promote stability.
  • Advocate on behalf of clients to ensure they have access to necessary services and entitlements, and make appropriate referrals to external agencies and organizations for specialized support as needed.
  • Provide crisis intervention and support to clients facing immediate challenges, such as eviction, substance abuse issues, mental health crises, or domestic violence situations.
  • Maintain accurate and up-to-date client records, including case notes, assessments, service plans, and progress reports, and ensure compliance with program requirements and funding regulations.
  • Train new case managers and enhance their skills in client engagement, assessment, service planning, and resource coordination.
  • Establish and maintain partnerships with other service providers, government agencies, housing authorities, and community organizations to enhance the range and quality of services available to clients.
  • Assist with monitoring the quality and effectiveness of case management services through regular review and implement improvements as needed to enhance outcomes for clients.
Requirements:
  • Bachelor's or Master's degree in social work, psychology, counseling, or a related field.
  • Previous experience in case management, preferably in homeless services or a related field.
  • Strong leadership, communication, and interpersonal skills.
  • Knowledge of community resources and social service agencies.
  • Ability to work effectively with individuals from diverse backgrounds and cultures.
  • Understanding of trauma-informed care principles.
  • Commitment to social justice and advocacy for marginalized populations.
Preferred Qualifications:
  • Ability to create positive, long-term working relationships by demonstrating a commitment to teamwork and customer service.
Additional Requirements:

In the event that an offer is extended, the candidate will be required to successfully complete a criminal background check and/or FBI fingerprinting, and pre-employment drug screening as applicable for the position.

This position is eligible for working remotely and is covered under the collective bargaining agreement.
