Nurse for Wellness Programs in Senior Living Facilities

Role Summary:
The Wellness Nurse plays a crucial role in supporting the Resident Care Director in the comprehensive execution, provision, and organization of Resident Care services within the community. This position also involves overseeing and managing the medication administration program.
Key Responsibilities:
The following essential functions represent the core duties of this role, which must be performed with or without reasonable accommodation:
- Supervise the medication management program and assist residents with their self-medication processes.
- Support residents with Activities of Daily Living (ADLs) and promote independence as needed.
- Foster positive relationships with residents, families, and healthcare providers.
- Report significant incidents or changes in residents' conditions to the Resident Care Director or Executive Director.
- Assist in conducting resident assessments and developing service plans.
- Participate in training assisted living staff and orient new team members.
- Engage in scheduled in-service training sessions and staff meetings.
- Help coordinate the scheduling and staffing of Resident Care Assistants.
- Availability for holidays and weekends is required.
- Perform additional duties as assigned.
- Maintain knowledge of and comply with relevant regulations for assisted living communities.
Qualifications:
- Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in good standing.
- Certification or licensure as per state regulations.
- Successful completion of all required training programs.
- Ability to apply common sense reasoning to various situations and follow detailed instructions.
- Strong interpersonal communication skills.
- Preferred candidates will have 1-3 years of experience in assisted living environments.

- Demonstrate a high level of service delivery and prioritize resident satisfaction while ensuring compliance with HIPAA regulations.
- Effectively communicate complex information verbally and listen attentively to residents and their families.

- Collaborate effectively with team members to achieve common goals.
- Exhibit tactfulness, maintain confidentiality, and promote an ethical workplace.
- Adapt to changing demands and manage transitions between tasks efficiently.
Physical Requirements:
- Capable of lifting at least 50 lbs. without assistance.
- Able to bend, reach, and work in confined spaces.
- Physically able to push and pull equipment and furnishings.
- Ability to stand and work on hands and knees for extended periods.
