Position Overview

The Case Manager will engage directly with families residing in Lighthouse's Emergency Family Shelter. Key responsibilities include delivering housing case management, linking families with vital resources, and facilitating their transition to stable, permanent housing.

Key Responsibilities

  • Conduct screenings and intakes for the shelter program
  • Manage a caseload of families
  • Provide nightly updates to the shelter team
  • Deliver intensive, weekly case management
  • Support operations at shelter locations
  • Complete asset and needs assessments
  • Screen and connect families to various housing programs
  • Assist families in obtaining affordable housing
  • Document timely case notes and service transactions
  • Support individuals transitioning out of the shelter
  • Encourage collaborative solutions across agencies
  • Ensure proper maintenance of client records
  • Provide emergency phone support and after-hours assistance

Required Qualifications

  • Personal or professional experience with individuals facing homelessness
  • Associate's degree
  • 1-2 years of experience in human/social services or case management
  • Ability to pass a background check
  • Reliable transportation
  • Proficient in Microsoft Office Suite
  • Adaptability and multitasking abilities
  • Experience working with diverse populations
  • Availability to work evenings and weekends

Preferred Qualifications

  • Bachelor's or master's degree in a relevant field
  • Over 2 years of experience in human/social services
  • Understanding of social factors affecting economic instability
  • Experience with HMIS
  • Ability to work independently while coordinating with others
  • Training in Harm Reduction, Trauma-Informed Care, Housing First, and SPDAT
  • Strong communication skills
  • High emotional intelligence
  • Integrity and ability to lead in a dynamic environment
