Ochsner Home Health Harahan is seeking a Full-Time Patient Care Management Assistant.
Previous experience in home health is preferred for LPN candidates.
The Patient Care Management Assistant plays a vital role in supporting the Patient Care Manager by coordinating a dedicated team to ensure the delivery of exceptional care to home health patients within the designated area. This position emphasizes the importance of skilled and informed staff committed to providing high-quality care in alignment with physician directives and State/Medicare regulations.
Key Responsibilities
- Facilitates communication with field staff, physician offices, referral sources, and patients, taking messages and processing orders as necessary.
- Regularly checks the status of incoming faxes and relays pertinent information to the appropriate team members.
- Coordinates the transmission of necessary documents to physician offices.
- Tracks pending laboratory results, ensures follow-up, and communicates findings to physicians and relevant personnel.
- Monitors hospitalized patients to guarantee effective care coordination.
- Assists the Patient Care Manager in managing various software reports that enhance care coordination and utilization management, promoting positive patient outcomes.
- Supports the Patient Care Manager with payer source authorizations as required.
- Conducts follow-ups in response to health check calls or tele-monitoring alerts, ensuring effective communication with team members.
- Prepares patient lists and necessary documentation for weekly case discussions.
- Provides direct patient care as needed, adhering to the scope of practice, care plans, and physician orders, while collaborating with other healthcare team members.
- Completes required training courses and additional assignments as directed by the Executive Director, including state-specific training as mandated.
- Assists the Patient Care Manager with clinician documentation reviews to ensure thorough, accurate, and timely submission of all documentation by field staff.
- Participates in the agency's Episode Management process.
- Performs other duties as assigned.
License Requirements
- Current CPR certification is mandatory.
- Valid LPN licensure in the state of practice is required.
- Possession of a current Driver's License, vehicle insurance, and access to reliable transportation or public transit.
- LA: Must hold a current LPN Licensure in LA without restrictions, with at least one year of experience as an LPN.
