Job Summary

We are seeking a highly skilled and compassionate Family Practice/Primary Care Nurse Practitioner to join our team at Fast Pace Health. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our physicians and staff to deliver exceptional patient outcomes.

Key Responsibilities:
  • Provide primary and preventive care to patients of all ages, including health promotion, disease prevention, and management of acute and chronic conditions.
  • Conduct thorough health assessments, develop and implement individualized care plans, and coordinate care with other healthcare professionals as needed.
  • Order and interpret diagnostic tests, prescribe medications, and provide education to patients and their families on healthy lifestyle choices and disease management.
  • Collaborate with our multidisciplinary team to ensure seamless care transitions and optimal patient outcomes.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices in family medicine and primary care.
Requirements:
  • Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree from an accredited program.
  • Current licensure as a Nurse Practitioner in the state of Louisiana.
  • Board certification as a Family Nurse Practitioner (FNP) or Primary Care Nurse Practitioner (PCNP).
  • Minimum of 2 years of experience as a Family Nurse Practitioner or Primary Care Nurse Practitioner in a fast-paced, outpatient setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
