Position Overview

Position Title: ECM Case Manager

Compensation: $21-$24 per hour

Supervisor: Director of ECM

Work Arrangement: Hybrid/Remote

Role Summary

The ECM Case Manager plays a crucial role in the clinical care coordination for clients enrolled in the ECM Program. This position involves the identification, organization, coordination, and monitoring of essential services required by clients. The Case Manager aids clients in accessing necessary services and community resources, primarily conducting activities in the field.

Qualifications
  • At least two years of experience working with homeless individuals and/or low-income populations, including those facing substance abuse and severe mental health challenges.
  • A Bachelor's Degree in a healthcare-related field is preferred.
  • Familiarity with community resources, housing options, and the government benefits system.
  • Strong verbal and written communication skills, with the ability to make informed clinical decisions regarding client care.
  • Must possess a valid driver's license and have access to a reliable, insured vehicle.
  • Current CPR certification is required.
  • Successful completion of a pre-employment physical examination, including TB screening or chest X-ray, as mandated by agency policies.
Key Responsibilities
  1. Coordinate with various stakeholders to ensure a seamless experience for clients and prevent service duplication.
  2. Engage with eligible clients to assess their needs.
  3. Conduct comprehensive bio-psycho-social assessments and develop individualized Care Plans, overseeing their implementation.
  4. Establish and maintain professional relationships with property owners, housing providers, and other stakeholders.
  5. Build trusting relationships with clients, focusing on promoting their independence and long-term housing stability.
  6. Facilitate access to mental health, substance abuse, and medical services, providing transportation as necessary.
  7. Ensure continuity of care by communicating effectively with healthcare providers.
  8. Connect clients to additional social services and supports, including transportation assistance.
  9. Advocate for clients with healthcare professionals to ensure their needs are met.
  10. Utilize motivational interviewing, trauma-informed care, and harm-reduction strategies in client interactions.
  11. Collaborate with hospital staff to develop discharge plans for clients.
  12. Accompany clients to appointments as needed, in accordance with organizational guidelines.
  13. Monitor adherence to treatment plans, including medication management.
  14. Maintain accurate documentation, including progress notes and case records for each client.
  15. Track interventions and outcomes to measure effectiveness.
  16. Address and resolve complaints from clients and their families.
  17. Report any signs of abuse or neglect in accordance with agency policies.
  18. Participate in training and quality improvement initiatives to ensure compliance with regulations and standards.
  19. Foster positive relationships with health plans and other service providers.
  20. Perform additional duties as assigned.
