Position SummaryThe ECM Case Manager plays a crucial role in overseeing the clinical care coordination for clients enrolled in the ECM Program. This involves a comprehensive approach to care that includes identifying, organizing, coordinating, and monitoring the necessary services for each recipient. The Case Manager is dedicated to assisting clients in accessing vital services and community resources, primarily working in the field.
Qualifications- At least two years of experience working with homeless, low-income, or mixed-income populations, particularly those facing substance abuse and severe mental health challenges.
- A Bachelor's Degree in a Health Care-related field is preferred.
- Familiarity with community resources, housing options, and the government benefits system.
- Strong verbal and written communication skills, with the ability to make informed clinical decisions regarding client care.
- Valid driver's license and access to a reliable, insured vehicle, meeting state and organizational standards.
- Current CPR certification is required.
- Successful completion of a pre-employment physical examination, including TB screening or chest X-ray, as mandated by agency policies.
- Coordinate with various individuals and organizations to ensure a seamless experience for clients while avoiding duplication of services.
- Engage with eligible clients effectively.
- Conduct comprehensive bio-psycho-social assessments and develop Care Plans, overseeing their implementation.
- Build and maintain professional relationships with property owners, managers, and housing providers.
- Establish trusting relationships with clients, focusing on promoting their independence and long-term housing stability.
- Facilitate access to mental health, substance abuse, and medical services, providing transportation when necessary.
- Communicate with healthcare providers to ensure continuity of care for clients.
- Connect clients to additional social services and support systems as needed.
- Advocate for clients with healthcare professionals.
- Utilize motivational interviewing, trauma-informed care, and harm-reduction strategies.
- Collaborate with hospital staff on discharge planning.
- Accompany clients to appointments as required, adhering to guidelines.
- Monitor adherence to treatment plans, including medication management.
- Ensure timely service delivery, maintaining regular contact with clients and conducting face-to-face visits as necessary.
- Maintain accurate documentation, including progress notes and case records for each client.
- Track interventions and outcomes effectively.
- Address complaints from clients, families, or friends professionally.
- Report any signs of abuse or neglect promptly.
- Participate in training, education, and quality improvement initiatives, assisting the agency in compliance with relevant regulations.
- Foster positive relationships with all health plans and vendors.
- Perform additional duties as assigned.
