We are seeking a compassionate and skilled Case Manager to join our team at the Mission Neighborhood Health Center. As a Case Manager, you will play a vital role in providing on-going case management and crisis counseling to our Resource Center participants.
Job Summary:The Case Manager will provide housing, immigration, and benefits support to our clients, as well as assist them in connecting with our treatment team and San Francisco medical and behavioral health system programs as needed.
Key Responsibilities:- Perform individual comprehensive assessments of Resource Center clients' psychosocial history, mental health status, medical needs, housing, food, and other basic support needs.
- Determine eligibility and necessity for case management, medical, and psychotherapy and psychiatric treatment, if appropriate.
- Develop care plans with clients, outlining steps necessary to accomplish activities, goals, and objectives.
- Implement care plans through the provision of interventions, coordination with integrated services teams, and facilitated referrals.
- Utilize a participant-centered, harm reduction model to provide supportive services to individuals, families, and groups of patients.
- Refer clients to appropriate shelter case management beds and assist them in finding, applying for, accessing, and stabilizing in housing.
- Assist clients in compiling documentation and filling out forms for immigration and public benefits' eligibility.
- Conduct home visits and attend appointments with clients, creating a hospitable environment and providing services with dignity and respect.
- Bachelor's Degree in Social Work, Psychology, Counseling, or a related field, and at least two years of paid work experience with homeless adults with multiple disabilities.
- Masters' Degree in Social Work (MSW) and/or Masters in Family and Child Therapist (MFT) degree preferred.
- Bilingual Spanish/English required.
- Knowledge of motivational interviewing, cognitive behavioral, and health psychology.
- Ability to work with de-escalation techniques, conflict resolution, and emergency response.
- Excellent interpersonal communication and writing skills.
- Ability to work professionally and ethically in a multi-cultural team setting, including maintaining a high level of confidentiality.
- Ability to work a flexible schedule, including evenings.
