Outreach Case Manager

PATH is seeking a dedicated and compassionate Outreach Case Manager to join our team in San Diego. As a key member of our Outreach Services team, you will play a vital role in helping individuals experiencing homelessness achieve stability and independence.
Job SummaryThe Outreach Case Manager will work closely with clients to assess their needs, develop individualized service plans, and provide supportive services to help them achieve their goals. This includes connecting clients with essential resources, such as healthcare, social services, and housing assistance.
Key Responsibilities- Conduct intake and needs assessments for clients, collaborating with them to develop individualized service plans that address housing goals, income, and other personal objectives.
- Provide referrals, linkages, information, and support to resources that help clients achieve their service plan goals.
- Complete program-specific assessments, such as the VI-SPDAT, to evaluate client strengths and challenges.
- Evaluate client progress and adjust service plans as needed, conducting regular reviews and updates.
- Maintain accurate and up-to-date client files, adhering to PATH's documentation standards and confidentiality guidelines.
- Two years of experience in healthcare, case management, homeless services, or a related field.
- Strong analytical and problem-solving skills, with the ability to think creatively and develop innovative solutions.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including clients, colleagues, and community partners.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Valid driver's license and reliable transportation, with the ability to qualify for PATH's insurance coverage.
