PATH is looking for dedicated individuals who are committed to making a significant impact in the lives of others to join our Interim Housing Services team as a Military Support Case Coordinator.

ABOUT PATH
Since its inception, PATH (People Assisting The Homeless) has led innovative and effective strategies to support individuals facing homelessness. PATH provides services across California, linking our clients to a comprehensive range of homelessness prevention, outreach, employment readiness, placement assistance, personalized case management, supportive services, interim housing, and permanent supportive housing.

ABOUT THE ROLE
The primary responsibility of the Case Coordinator is to aid clients in overcoming homelessness through a comprehensive and adaptable approach. This position guides clients through various stages of homelessness, including but not limited to: street outreach, interim housing, transitional housing, and permanent housing. The Case Coordinator assists clients in accessing and sustaining essential services related to healthcare, social services, personal support systems, and navigating a coordinated entry system. This role will deliver tailored supportive services by helping each client formulate a strategy to address their strengths, challenges, health needs, enhance income, and secure and maintain housing. As part of the care plan, the Case Coordinator identifies the areas where clients require assistance to achieve their defined goals and objectives (e.g., scheduling appointments, applying for public benefits, etc.) and emphasizes housing and health as critical outcomes for success. Key Responsibilities include:

Conducting intake and personalized needs assessments for all clients and collaborating to create Individualized Service Plans (ISP) that focus on housing objectives, income maintenance/increase, and other personal aspirations identified by the client, including medical, mental health, substance use, financial resources, vocational, and social support needs. Providing referrals, connections, information, and support to resources that assist clients in achieving their ISP objectives. Completing program-specific assessments (e.g., VI-SPDAT). Evaluating strengths and challenges in addressing both short-term and long-term goals, conducting a review with the client after the initial intake.

QUALIFICATIONS

Effective Communication: Clearly and effectively convey information (both verbally and in writing) to individuals and groups in a manner that informs, engages, and maintains their attention. Creativity and Initiative: Proactively take action and make decisions without being prompted, demonstrating a willingness to assume additional responsibilities. Think innovatively, generating creative solutions to challenges. Critical Thinking and Problem Solving: Analyze and evaluate information and situations, identify key issues, consider alternatives, and make logical decisions based on sound reasoning to achieve desired outcomes. Ethical Standards and Boundaries: Uphold ethical and professional standards, demonstrating honesty, integrity, and a commitment to doing the right thing while maintaining professional boundaries.

PREFERRED EXPERIENCE

Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations or a related field.

MINIMUM EXPERIENCE

All levels of education and experience are welcome.

MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:

Have employment eligibility verification. Have or be willing to obtain CPR/First Aid training. Successfully complete the following as a condition of hire: Tuberculosis Test, Background Screening, Drug Test.

DRIVING REQUIREMENTS
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:

Have reliable transportation. A valid driver's license. Proof of insurance and ownership for personal vehicles used during work duties. The ability to qualify for PATH's insurance coverage.

EMPLOYMENT ELIGIBILITY

FLEXIBLE WORK
As a California employer, all regular and customary work must be performed within the state. Approval for work outside of the office does not establish approval for work to be conducted outside of California.
