Position Purpose:

The HSS Case Manager plays a crucial role in delivering high-quality services to seniors aged 55 and above. This position involves providing tailored social services, health care, and support to enhance the daily living of older adults.

Key Responsibilities:

  • Deliver intensive case management services from initial contact through to housing placement.
  • Collaborate with a multidisciplinary team to ensure comprehensive client support.
  • Facilitate the transition of clients from homelessness to stable housing.
  • Develop expertise in the Coordinated Entry System (CES) and related tools.
  • Engage with property management to resolve tenancy issues and prevent evictions.

Essential Duties:

  • Connect clients with both internal and external resources.
  • Act as a liaison between clients and community service providers.
  • Maintain updated listings of community resources.
  • Provide advocacy and outreach to enhance community engagement.
  • Manage a caseload effectively, balancing walk-in and scheduled appointments.
  • Develop and implement individualized care plans for clients.
  • Conduct assessments as necessary.
  • Represent the organization at community meetings focused on senior issues.
  • Ensure timely and accurate documentation of client interactions and referrals.

Qualifications:

A Bachelor's degree is preferred, or an equivalent combination of education and experience relevant to the role. A background in social work is advantageous. Bilingual skills in Spanish are preferred.

Compensation: $23 to $25 per hour, depending on experience.
