Patient Care Director

We are seeking a highly skilled and experienced Patient Care Manager to join our team at Heartland Home Health. As a Patient Care Manager, you will play a critical role in ensuring the delivery of high-quality patient care and services to our clients.
Key Responsibilities- Clinical Operations: Oversee the day-to-day clinical operations of our home health sites, ensuring that patients receive individualized and appropriate care.
- Quality Assurance: Ensure that all patients' plans of care are executed as written and reassessed by the appropriate healthcare professional when there is a significant change in the patient's condition.
- Documentation: Ensure that all patients transferred to an inpatient facility have completed documentation.
- Site Management: Manage the site in the absence of the Executive Director or Administrator.
- Clinical and Administrative Functions: Oversee clinical and administrative functions, including recordkeeping.
- On-Call Nursing: Participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes.
- Education: Associate's degree in Nursing from an accredited school of nursing. Bachelor's degree in nursing from an accredited school of nursing preferred.
- Experience: Minimum three years of experience in direct patient care in a clinical setting.
- Licensure and Certification: Current RN licensure in the state of practice and current CPR certification.
- Skills: Ability to effectively operate nursing assessment tools and office equipment, including computers and tablets.
