Patient Care Director

We are seeking a highly skilled and experienced Patient Care Director to join our team at Heartland Home Health. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of our home health program, ensuring the delivery of high-quality patient care and services.
Key Responsibilities- Program Management: Develop and implement effective strategies to improve patient outcomes, increase patient satisfaction, and enhance the overall quality of care.
- Clinical Oversight: Provide clinical guidance and support to our nursing staff, ensuring that patients receive evidence-based care and services.
- Operations Management: Manage the administrative and operational aspects of our home health program, including staff supervision, budgeting, and resource allocation.
- Collaboration and Communication: Foster strong relationships with our referral sources, patients, caregivers, and employees to ensure seamless communication and coordination of care.
- Education: Associate's degree in Nursing from an accredited school of nursing. Bachelor's degree in nursing preferred.
- Experience: Minimum three years of experience in direct patient care in a clinical setting. Current RN licensure in the state of practice and current CPR certification required.
- Skills: Strong leadership and management skills, excellent communication and interpersonal skills, ability to operate nursing assessment tools and office equipment, and proficiency in computer and tablet technology.
