Key Responsibilities:

  • Conduct the RAI process, performing assessments and coordinating care plans for assigned residents.
  • Complete the MDS, CAAs, and care plans within the mandated time frames.
  • Evaluate residents through physical assessments, interviews, and chart reviews.
  • Engage with caregivers, including physicians, nursing staff, social services, therapy, dietary, and activities personnel, to discuss resident care needs.
  • Review information from hospitals and external agencies, integrating this data into assessments and care planning.
  • Identify and initiate significant changes in MDS as necessary.
  • Prepare for and conduct PPS meetings, ensuring MDS assessments are maintained according to Medicare schedules, and manage the PPS board for monitoring Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
  • Stay updated with the requirements set forth by the American Association of Nursing Assessment Coordinators (AANAC).

Qualifications:

  • Must be a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
  • AANC certification is advantageous, particularly RAC-CT.
  • Familiarity with the Resident Assessment Instrument (RAI) process and the principles of the Prospective Payment System (PPS) is highly preferred.
  • Prior experience as an MDS Nurse is essential.
