Key Responsibilities:
The MDS Nurse plays a crucial role in the RAI process, conducting assessments and coordinating care plans for assigned residents. Specific duties include:

  • Completing the MDS, CAAs, and care plans within mandated timelines.
  • Performing resident assessments through physical evaluations, interviews, and chart reviews.
  • Collaborating with caregivers, including physicians, nursing staff, social services, therapy, dietary, and activity personnel to address resident care needs.
  • Reviewing information from hospitals and external agencies to inform assessment and care planning.
  • Coordinating and initiating significant changes in MDS documentation.
  • Preparing for and conducting PPS meetings, ensuring MDS assessments align with Medicare schedules, and maintaining oversight of Medicare days and RUGs utilization.
  • Staying updated with the requirements set forth by the American Association of Nursing Assessment Coordinators (AANAC).


Qualifications:
To be considered for this role, candidates must possess:

  • A valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
  • AANC certification is advantageous, particularly RAC-CT.
  • Familiarity with the Resident Assessment Instrument (RAI) process and the principles of the Prospective Payment System (PPS) is highly preferred.
  • Prior experience as an MDS Nurse.
