We are seeking a highly skilled MDS Coordinator to join our team at Richmond Healthcare & Rehab Center. As an MDS Coordinator, you will play a critical role in ensuring that our documentation meets Federal, State, and Certification guidelines.
Key Responsibilities- Regulatory Compliance: Assist the center in ensuring adherence to Federal and State regulations and certification.
- RAI Process Coordination: Coordinate the RAI process, ensuring timeliness and completeness of the MDS, CAAs, and Interdisciplinary Care Plan.
- Quality Assurance: Participate in the regulatory or certification survey process and report trends from completed audits to the Quality Assurance Committee.
- Process Improvement: Identify areas for improvement and implement changes to enhance the quality of care and documentation.
- Technology Integration: Utilize the PointClickCare system to initiate and monitor RAI process tracking, discharge/reentry, and Medicaid tracking forms.
- Staff Support: Provide guidance and support to staff to ensure compliance with standards of documentation.
- Assessment and Data Collection: Complete patient assessments, data collection, and interviews with staff as necessary to ensure good standards of practice.
- Education: Graduate of an approved Registered Nurse program and licensed in the state of practice.
- Experience: Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
- Knowledge and Skills: Excellent knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement. Through understanding of the Quality Indicator process, OBRA regulations, and Minimum Data Set. Knowledge of the care planning process.
