Position Overview:

We are seeking a Clinical Care Coordination Specialist to join our team.

Key Responsibilities:

  • Conduct triage to assist RN/NP in evaluating patients presenting with both active and inactive symptoms.
  • Operate within the scope of certification under professional guidance, including obtaining vital signs, performing EKGs, assessing symptoms, and providing assistance as directed by providers.
  • Gather clinical information from patients to support Physicians and Nurse Practitioners in making clinical decisions and developing care plans.
  • Execute testing orders and secure necessary signatures from the ordering provider.
  • Complete various forms such as FMLA, disability, and others, ensuring provider signatures are obtained.
  • Communicate with patients regarding standard Coumadin instructions.
  • Assist RN in collecting clinical information for patients with abnormal INR levels as per guidelines.
  • Document all patient interactions in the electronic medical record system.
  • Collaborate with IT for any EMR-related issues, including discrepancies in test/lab results.
  • Work with external laboratories to ensure lab results are accurately imported into the EMR.
  • Monitor shared nursing calendars to ensure timely completion of lab work and integration into the EMR.
  • Engage with third-party representatives to ensure effective collaboration.
  • Contact laboratories to provide necessary ICD 10 codes for testing ordered by providers.
  • Assist Physicians and Nurse Practitioners in placing and modifying orders for home nursing, infusion services, and hospice care, ensuring all required signatures are obtained.
  • Act as a liaison for care team staff, providing assistance with inquiries and workflow processes.
  • Manage orders for mobile phlebotomy services and monitor expiration dates.
  • Distribute biweekly overdue INR lists to care team members.
  • Sort clinical faxes as needed within the call center.
  • Facilitate communication regarding nurse/care team schedules.
  • Support the training of new care team staff members.

Qualifications:

  • Certification as a Medical Assistant is required.
  • A minimum of 5 years of experience in the field of Cardiology is essential.
  • Ability to multitask effectively in a fast-paced environment.
  • Proficient computer skills are necessary.
  • Strong communication skills, with a friendly and approachable demeanor.
