Position Overview:
The role is pivotal in ensuring that clients transition from treatment with a comprehensive aftercare strategy, secure housing, and any additional necessary support. This position also contributes to achieving overall census and financial objectives.

Key Responsibilities:

• Deliver high-quality direct and indirect care to individuals facing substance use or dependency challenges.

• Evaluate clients' ongoing care and referral requirements.

• Build strong relationships with clients.

• Gather essential information and obtain necessary consents from clients, families, referral sources, and other relevant entities.

• Facilitate referrals to Mental Health and Substance Abuse treatment facilities for ongoing care.

• Coordinate interviews and placements in Halfway Houses.

• Collaborate closely with counselors and members of the multidisciplinary team.

• Attend and contribute to program, clinical, and administrative meetings.

• Actively support the facility's referral initiatives.

• Participate in mandatory training and orientation sessions.

• Perform other assigned duties as required.

Qualifications:
Education & Experience
• A High School diploma is required; a Bachelor's Degree in a human services-related field is preferred.

Knowledge, Skills, and Abilities:
To excel in this role, candidates must demonstrate the ability to perform essential duties effectively. The following qualifications are essential:

• Strong oral and written communication skills and interpersonal abilities.

• Familiarity with clinical care levels, primary funding sources, and referral networks.

• Competence in problem-solving through information gathering and analysis.

• Ability to manage crisis situations with appropriate responses.

• Understanding of federal and state regulations, as well as standards set by regulatory bodies.

Physical Requirements:
While executing the responsibilities of this position, the employee frequently engages in verbal communication and listening. Regular tasks include sitting, using hands for manipulation, and reaching. Occasionally, the employee may need to climb stairs, stand, walk, bend, twist, kneel, or crouch. The employee must be able to lift and/or move up to 25 pounds regularly and occasionally lift and/or move up to 50 pounds. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

Work Environment:
The work environment is typically moderately quiet and indoors. Scheduling will be based on operational needs, and employees are expected to provide their own transportation. Employees may encounter exposure to viruses, diseases, and infections from clients, as well as potentially traumatic situations.
