About the Role

This Care Coordination Specialist role is an exciting opportunity to make a meaningful impact in the lives of our patients. As part of our interdisciplinary team, you'll play a critical role in ensuring seamless transitions between levels of care, promoting efficient resource utilization, and facilitating timely discharges.

You will serve as the primary liaison between the healthcare team and patients/caregivers, providing education and support throughout the care continuum. Your expertise in case management and discharge planning will be invaluable in coordinating with external payers, attending physicians, and other stakeholders to achieve optimal outcomes.

Key Responsibilities
  1. Manage a caseload of patients, prioritizing those with complex needs and developing individualized plans for successful transitions.
  2. Communicate effectively with patients, families, and healthcare professionals to ensure clear understanding of treatment options, financial responsibilities, and post-acute care arrangements.
  3. Collaborate with the interdisciplinary team to identify opportunities for improved resource allocation, streamlining processes, and enhancing patient satisfaction.
  4. Facilitate effective communication among team members, providers, and payers to resolve issues related to authorizations, denials, and payment discrepancies.
Requirements

To be successful in this role, you must possess:

  • A Master's degree in Social Work (MSW) or equivalent experience.
  • A valid LSW/LCSW license in the state of Illinois.
  • Proven experience in case management and/or discharge planning within a hospital setting.
  • Strong analytical, problem-solving, and communication skills, with the ability to work effectively in a fast-paced environment.
