Position Summary

The Care Coordination Manager plays a crucial role in bridging gaps in patient care by acting as a vital resource for clinic leadership and healthcare providers. This position is responsible for identifying patients who require services and overseeing the scheduling and planning of care, which encompasses annual examinations, follow-ups from emergency room visits, and collaborative care initiatives.

Key Responsibilities

  • Oversee staff involved in patient care coordination, including appointment scheduling, reminder communications, and transportation arrangements.
  • Identify and prioritize patients with significant care gaps, effectively communicating these needs to clinic leadership and providers, while ensuring follow-up on actions taken.
  • Establish and maintain an efficient system for tracking, reporting, and evaluating the management of care gaps and outcomes for all stakeholders.
  • Develop and utilize dashboards and scorecards to monitor and report on care coordination outcomes.
  • Work collaboratively with relevant personnel to ensure accurate and timely documentation of patient care.
  • Assist clinic leadership and providers in managing patient appointments and scheduling to optimize clinician productivity and enhance patient satisfaction.
  • Provide exceptional customer service and foster relationships with referral providers to support business objectives.
  • Ensure that all patients referred to the organization are contacted for appointments within the timeframe set by insurers after discharge from referring facilities.
  • Coordinate inquiries from patients, referral sources, payors, and colleagues to provide timely and accurate responses.
  • Facilitate referrals to healthcare providers and offer educational services as part of outreach efforts to monitor patient care gaps.
  • Track performance metrics related to patient care coordination, including registration, care gaps, referrals, and outreach initiatives.
  • Prepare and distribute reports on clinical care and patient care coordination outcomes as directed by the Clinical Services Director.
  • Identify opportunities for performance improvement, track trends, and report progress regularly, collaborating with providers and clinic leadership on clinical outcomes.

Qualifications

  • High School Diploma or GED required; an Associate degree in business, healthcare administration, or a related field is preferred.
  • A minimum of 5 years of experience in patient scheduling or a healthcare environment is required.
  • Strong verbal and written communication skills are essential for effective interaction with physicians, therapists, community agencies, staff, and patients.
  • Proficiency in computer applications, including Word, Excel, and Outlook, is necessary.
  • Ability to maintain confidentiality is critical.
  • Availability to work evenings and weekends as needed by management.
