Position Overview

The MSW Social Services Coordinator plays a crucial role in enhancing the well-being of participants and their families through effective social work services. Under the guidance of the Center Administrator, this position is responsible for the strategic planning, organization, and execution of social work initiatives tailored to the needs of PACE participants.

Key Responsibilities
  • Conduct comprehensive assessments and provide therapeutic interventions to participants and their caregivers.
  • Facilitate education and counseling sessions for families, ensuring they are well-informed and supported.
  • Offer resources and support to address mental health concerns as they arise.
  • Manage ongoing cases and advocate for the needs of participants and their caregivers.
  • Serve as a vital link between the interdisciplinary team, caregiver representatives, and community organizations.

Prior to engaging with participants, it is essential to obtain medical clearance for communicable diseases and maintain current immunizations. A valid driver's license, proof of insurance, and reliable transportation are also required, adhering to EveryAge's auto insurance standards.

Qualifications

Education:
Master's degree in Social Work from an accredited institution is mandatory. Membership in the Academy of Certified Social Workers (ACSW) or other recognized NASW certifications is preferred. CPR and First Aid certifications are required.

Experience:
A minimum of two years working within an interdisciplinary team in healthcare settings such as hospitals, nursing homes, or community-based environments is preferred. Additionally, at least one year of experience working with the elderly or frail population is essential.
