Job Summary

We are seeking a highly motivated and compassionate Case Manager/Service Coordinator to join our team. As a key member of our care team, you will be responsible for providing case management activities necessary to meet the needs of our clients. This includes developing individualized care management plans, implementing care plans through brokering and coordinating services, and monitoring and evaluating clinical outcomes to ensure services are provided in a professional, comprehensive, and cost-effective manner.

Key Responsibilities

  • Complete assessments to identify client needs and desires for service delivery
  • Develop and modify Person-Centered Service Plans annually or as needed
  • Ensure client choice of providers through informed decision-making
  • Coordinate with other care coordinators and entities to ensure seamless care delivery
  • Research and develop alternative solutions to client needs
  • Ensure confidentiality and maintain accurate records
  • Attend required trainings and recertification classes

Requirements

  • BS/BA degree in related coursework
  • Minimum 3 years of experience in case management for individuals with intellectual disabilities, autism spectrum disorders, or physical disabilities
  • Experience with Microsoft Office and state-maintained databases
  • Maintain proficiency in company-sponsored training and certifications
  • CPR/First Aid certification and updated state-related clearances
