Social Work Care Manager-MSW


At EveryAge, we are seeking a skilled Social Work Care Manager to join our interdisciplinary team. As a key member of our team, you will be responsible for planning, organizing, and implementing social work services to our PACE participants and their families. Your responsibilities will include assessment, treatment, teaching, and counseling to participants, caregivers, or other representatives. You will also provide resources, address mental health needs, and ensure participant and caregiver needs are met and addressed. Additionally, you will serve as a liaison between our interdisciplinary team, caregiver representatives, and community agencies.
Requirements include a Master's degree in Social Work from an accredited school of social work, membership in the Academy of Certified Social Workers (ACSW) or other NASW-recognized certification, and Cardio Pulmonary Resuscitation (CPR) and First Aid certification. Experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting is preferred, as well as a minimum of 1 year working with the frail or elderly.

Responsibilities
