Key Responsibilities:
  • Complete departmental orientation and initial and annual competencies
  • Collaborate with other departmental staff to enhance patient care delivery, identify barriers to care or discharge, and devise solutions
  • Ensure documentation is completed according to workflow timelines and content requirements, including the Individual Plan of Care (IPoC) as per CMS guidelines
  • Organize family conferences and maintain communication with caregivers following team meetings to keep them informed of patient progress and rehabilitation plans
  • Facilitate weekly patient care team conferences to develop, monitor, and refine treatment plans aimed at achieving patient goals and outcomes
  • Review assigned Case Mix Groups (CMG) and assist the team in identifying any potential comorbid conditions being treated during the patient's stay, communicating findings to the Health Information Management (HIM) team
  • Perform additional duties as assigned

Qualifications:
  • Current licensure as a Registered Nurse, Social Worker, or other healthcare professional such as a Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist
  • Preferred certifications in Case Management or Rehabilitation Nursing, such as those from the Commission for Case Manager Certification (CCM), Association of Rehabilitation Nurses (ARN), American Case Management Association (ACM), or Board Certification in CM by the ANCC
  • A minimum of 2 years of experience in social work or case management within an inpatient setting is highly preferred; experience in acute or rehabilitation hospitals is a plus
  • Strong oral and written communication skills in English; proficiency in additional languages is advantageous
  • Basic computer skills in applications such as Excel, Word, Outlook, and PowerPoint are required

Compensation ranges from $41 to $56 per hour. While we anticipate that new hires will be compensated within this range, certain circumstances may warrant deviations.
