Job Summary

Aveanna Healthcare is seeking a skilled Registered Nurse to join our team as a Skilled Care Specialist. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Deliver Quality Patient Care: Provide skilled nursing care to patients in their homes, adhering to established care plans and physician orders.
  • Coordinate Care Services: Collaborate with healthcare professionals to ensure seamless care coordination and effective communication.
  • Assess Patient Needs: Continuously assess patient condition and care needs, reporting changes to supervisors and physicians as necessary.
  • Document Patient Care: Accurately document all patient interactions, assessments, and care provided in a timely manner.
  • Develop and Implement Care Plans: Develop, implement, and update individualized care plans to meet patient needs and achieve desired outcomes.
  • Provide Education and Support: Educate patients and families on disease management, self-care techniques, and prevention strategies to promote optimal health outcomes.
Requirements
  • Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Licensure: Current, unrestricted state license as a Registered Nurse in the state of practice.
  • Certifications: Current CPR certification and demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies.
  • Experience: One year of experience working under current nursing license, with a preference for six months of recent experience as a Licensed Nurse in a clinical care setting.
Preferred Qualifications
  • Home Health Experience: Previous experience in home health care settings.
  • Specialized Skills: Strong attention to detail, effective problem-solving and conflict resolution, and good organization and communication skills.
Physical and Environmental Demands
  • Physical Requirements: Ability to speak, write, read, and understand English, with the ability to travel and lift 50 pounds.
  • Environmental Demands: Ability to function in a wide variety of environments, with possible exposure to allergens and infectious diseases.
