Position Overview

We are offering a $7,500 hiring bonus for qualified external candidates who meet the necessary qualifications and conditions for payment.

The Primary Care Manager RN is responsible for delivering comprehensive care management, coordination, and skilled nursing services to at-risk elderly patients facing complex medical and psychosocial challenges. This role utilizes population-based disease management strategies and implements clinical protocols as essential components of the program. As a vital member of the patient-centered PACE interdisciplinary team (IDT), the RN's primary objective is to ensure a systematic approach to coordinating health services throughout the care continuum, enhancing patient experiences, managing health expenditures, and improving overall quality of care.

Our PACE program is dedicated to maintaining the health and well-being of older adults in the community through various services, including clinics and home visits. This position empowers elders to lead active and independent lives.

Essential Qualifications:

  • Oregon Registered Nurse License upon hire
  • National Provider BLS certification from the American Heart Association within 30 days of hire
  • A minimum of 2 years of RN experience in PACE, home health, skilled nursing facilities, community health, hospice nursing, or related fields

Preferred Qualifications:

  • Bachelor's Degree in Nursing
  • Current recognition by a national professional organization in care management, geriatrics, or hospice upon hire
