Clinical Care Manager RN

Providence Portland Medical Center is seeking a Clinical Care Manager RN to join our team. As a Clinical Care Manager RN, you will be responsible for leading the care management team and ensuring the delivery of high-quality patient care.
Key Responsibilities:- Lead the care management team and ensure the delivery of high-quality patient care.
- Develop and implement care management plans that meet the needs of patients and families.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Provide education and support to patients and families to promote self-care and independence.
- Conduct regular assessments and evaluations to ensure the effectiveness of care management plans.
- Bachelor's Degree in Nursing.
- Current Oregon Registered Nurse License.
- 2 years of supervisory experience, including program planning, performance management, staff development, and evaluation.
- 1 year of care management experience.
- 3 years of relevant experience in one or more of the following areas: social services, utilization review/quality management, case management.
- Master's Degree in Nursing.
