RN - PACE/Elderplace Care Specialist

We are seeking a compassionate and skilled Registered Nurse (RN) to join our Providence PACE team in Milwaukie, Oregon. As a Community Care RN, you will play a vital role in providing high-quality, patient-centered care to older adults in their homes and communities.
Key Responsibilities- Provide and supervise professional, independent nursing care for Providence patients, based on the nursing process and consistent with the Oregon Nurse Practice Act.
- Apply knowledge of assessing, planning, ordering, giving, delegating, teaching, and supervising professional nursing care delivery that promotes patients' optimal health and independence.
- Develop and validate knowledge and skills essential to the assigned department, patient population, and/or practice area.
- Rotate through a weekend shift, as required.
- Oregon Registered Nurse License
- National Provider BLS - American Heart Association (within 30 days of hire)
- Relevant certifications applicable to the position, as specified by the Nurse Manager (upon hire)
- Bachelor's Degree in Nursing
- 1 year Home health RN experience
