Care Management Department Overview

The Care Management Department at Providence Portland Medical Center is responsible for the direction and management of the inpatient care management model. This position provides leadership and oversight for the clinical care management model, staff of approximately 24-31 FTE, budget of approximately $M, and operations of the care management department for the facility.

Key Responsibilities

  • Provide leadership and oversight for the clinical care management model
  • Manage a team of approximately 24-31 FTE
  • Oversee the budget of approximately $M
  • Ensure the effective operation of the care management department

Requirements

  • Bachelor's Degree in Nursing
  • Oregon Registered Nurse License
  • National Provider BLS - American Heart Association
  • 2 years Supervisory experience, including program planning, performance management, staff development and evaluation
  • 1 year Care Management experience
  • 3 years Relevant experience in one or more of the following areas: Social Services, Utilization Review/Quality Management, Case Management
