About the Role

The Patient Services Coordinator II plays a vital role in delivering exceptional patient care at Mount Sinai Health System. As a liaison between patients, physicians, and clinical staff, you will ensure seamless coordination of patient care activities, prioritizing work to meet the needs of patients, families, and caregivers.

Key Responsibilities
  1. Collaborate with Primary Care Providers to facilitate patient care plans, participating in daily huddles and monthly case discussions to ensure continuity of care.
  2. Work closely with clinical staff to resolve issues impacting patient care progression, ensuring timely and effective resolution.
  3. Monitor and update dashboards with reminders and alerts to organize daily tasks, including visit status reminders, follow-up appointments, and authorization for medical review.
  4. Contact insurance carriers for pre-certification and consult with Department/Practice Manager to address patient needs or escalated issues.
  5. Provide front-end coverage as needed, recording telephone encounters for routing to appropriate Providers.
  6. Act as a liaison between Physicians, patients, and clinical staff to connect patients with relevant internal/external resources.
  7. Recommend process/service improvements to promote patient satisfaction and address issues.
  8. Provide thoughtful and informative responses to patients, guiding them through their healthcare options.
  9. Maintain understanding of medical terminology and practices used within the hospital through further education.
Qualifications
