We are seeking a highly skilled and compassionate Senior Case Manager to join our team at Services for the UnderServed (S:US). This is a critical role that requires a strong understanding of social services, case management, and community outreach.
Key Responsibilities- Conduct intake processes with new clients, ensuring timely and accurate documentation of client information and services provided.
- Develop and implement individualized service plans, monitoring client progress and making adjustments as needed.
- Provide comprehensive case management services, including referrals to medical, mental health, and supportive social services.
- Collaborate with internal and external stakeholders to ensure seamless service delivery and client outcomes.
- Maintain accurate and up-to-date client chart documentation, adhering to SUS standards and regulations.
- Bachelor's degree in a Human Services field.
- At least 3 years of experience working with homeless populations, preferably in a residential setting.
- Ability to work with a culturally diverse population and maintain confidentiality.
- Master's degree in a Human Services field.
- Familiarity with NYC public school systems and community resources.
- Experience in residential settings, group work, tenant organization, case management, referral, and/or community outreach.
