Senior Case Manager

The Senior Case Manager will provide direct service provision to residents and supervision of the Recreational Specialist or Residential Specialist according to the site's staffing pattern. This role is responsible for conducting intake interviews, assessing and evaluating residents for admission, and engaging residents in initial and ongoing treatment processes through individual and group counseling.
Key Responsibilities
• Maintain a caseload of 20-25 families
• Conduct intake interviews and assess residents for admission
• Engage residents in treatment processes through individual and group counseling
• Develop and maintain housing referral resources and assist residents in securing permanent housing
• Assess residents' vocational and educational needs and develop placement and follow-up plans
• Develop and maintain liaison activities with DHS, HRA, and other community agencies to assist eligible residents in securing public assistance and Medicaid
Requirements
• Bachelor's Degree in Human Services
• Three years of crisis counseling experience in a residential shelter for homeless individuals and/or families
• Training and experience in assessing vocational and educational needs and aptitudes of clients
• Familiarity with DHS regulations and DHS Cares Reporting
• Excellent written and verbal communication skills
