Job Summary

We are seeking a highly skilled Senior Case Manager to join our team at Services for the UnderServed. The successful candidate will be responsible for ensuring quality service delivery to individuals and families in need of homeless prevention services.

This is a challenging and rewarding role that requires strong case management skills, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment.

The Senior Case Manager will provide supervision to case managers, oversee intensive case management for program participants, and ensure community follow-up to engage participants in care. They will also monitor and evaluate program participants' progress, assure timely intakes, assessments, and referrals, and promote linkage development and monitoring.

Key Responsibilities:

  • Provide supervision to case managers and oversee intensive case management for program participants
  • Ensure community follow-up to engage participants in care and promote compliance with appointments
  • Monitor and evaluate program participants' progress and assure timely intakes, assessments, and referrals
  • Promote linkage development and monitoring with other on-island service providers
  • Prepare and submit Financial Assistance Requests on behalf of program participants
  • Participate in development of After-Care and on-going housing assistance service planning processes
  • Coordinate supportive services and referrals based on specific needs of program participants

Requirements:

  • Master's Degree or BA/BS enrolled in accredited Master's Degree program with two years relevant experience
  • Knowledge of the Department of Homeless Services, the Human Resources Administration, and entitlements
  • Strong computer abilities, including Microsoft applications and data input
