Housing Case Manager

Upward Bound House is seeking a highly motivated and compassionate Housing Case Manager to join our team. As a Housing Case Manager, you will play a critical role in providing housing-focused case management services and rental assistance to Transition Age Youth (TAY) and young adults experiencing homelessness.
Job Summary:The Housing Case Manager will provide field-based, housing-focused case management and supportive services to TAY and young adults experiencing homelessness. This includes unit identification, landlord engagement and mediation, time-limited rental assistance, and housing stability assistance.
Key Responsibilities:- Outreach to TAY and young adults experiencing homelessness to screen for the Youth Homelessness Demonstration Program
- Conduct comprehensive intake and assessment of participants and develop a Housing Service Plan in collaboration with program participants
- Assist participants in locating and securing permanent housing, including identifying housing resources, communicating with landlords, and processing applications
- Conduct regular home visits to ensure housing stability standards are maintained and to support housing retention
- Educate participants on tenant rights and responsibilities, budgeting, and money management for housing expenses
- Network with landlords and property managers to discuss UBH's rental assistance programs and encourage their participation
- Perform inspections of units for habitability standards and negotiate rent when necessary
- Provide advocacy as needed to help address issues and barriers between landlords and participants
- Assist participants in achieving wellness and autonomy through case management services, including providing resources, tools, and counseling, and making referrals for services
- Bachelor's Degree in Human Services or related field
- Minimum of 2 years' experience working with unhoused populations, with a preference for experience working with TAY
- Demonstrated knowledge of fair housing laws and practices
- Exhibit high level of professionalism and ability to maintain personal and professional boundaries
- Strong written and verbal communication skills
- Ability to respond responsibly in emergency and crisis situations
- Intermediate proficiency in Windows Office Suite programs and a data management system (HMIS preferred)
The Housing Case Manager will work in a fast-paced environment, interacting with diverse populations, and may be exposed to unpleasant sensory demands associated with working with individuals and families in crisis. The employee must be able to walk, climb stairs, and drive a personal vehicle in and around Los Angeles County.
