The Foundations for Families initiative offers a range of services including home-based treatment, community outreach, thorough assessments, counseling, case management, and crisis intervention for youths and families engaged with the Department of Mental Health, Department of Children and Families, and Mass Health systems. Our services are tailored to meet the unique needs of each family, ensuring a personalized approach.

Our team is composed of highly trained professionals skilled in family engagement, child and adolescent development, and crisis management. Depending on the service required, our staff may include licensed clinicians, master's level professionals, parent partners with lived experience, and bachelor-level therapeutic support staff.

Services are flexible and can be provided on a scheduled basis or in response to crises, focusing on maximizing the strengths of the youth and family while maintaining family unity and preventing out-of-home placements. Additionally, we ensure families receive comprehensive information and referrals to community resources to enhance their support networks.

The Foundations for Families services can function independently or be integrated into broader service plans that involve other providers such as hospitals, state agencies, and community programs.

Compensation: $24/hour

Key Responsibilities:

  • Conduct intake assessments for new program referrals.
  • Educate and assist caregivers in navigating care systems and accessing appropriate services for their children.
  • Support caregivers in developing problem-solving and advocacy skills.
  • Help locate and access necessary resources.
  • Guide caregivers in self-care strategies and respite care options.
  • Provide real-time modeling and coaching to enhance parenting skills and confidence.
  • Collaborate with youth, families, and caregivers to create strength-based assessments and treatment plans.
  • Deliver individual and family therapeutic support as outlined in the service plan.
  • Work in partnership with clinicians and other treatment team members.
  • Facilitate or participate in case conferences and treatment meetings as needed.
  • Document progress notes, aftercare plans, and service plans as required.
  • Transport clients to appointments and home visits as necessary.
  • Assist colleagues in crisis prevention and management.
  • Adhere to all agency policies and procedures in performing duties.
Qualifications:
  • Must have personal experience as a caregiver for a youth with significant social, emotional, behavioral health, medical, or trauma-related needs and familiarity with child and family service systems.
  • Bachelor's degree preferred.
  • Availability for flexible scheduling, including evenings and weekends.
  • Strong interpersonal, organizational, and writing skills are essential.
  • Valid driver's license required for travel.
Full-time employees are eligible for benefits including medical, dental, and vision insurance, retirement plans, three weeks of paid vacation, and tuition reimbursement up to $2500 per fiscal year.
