Job Title: Case Manager/Family Support Worker

NFI Massachusetts Inc., a nonprofit human service agency, is seeking a skilled Case Manager/Family Support Worker to join its Foundations for Families program in the North Shore. The ideal candidate will have lived experience as a parent/caregiver of a youth with significant social, emotional, behavioral health, medical or trauma related needs and expertise in navigating child and family serving systems.

Job Summary:

The Case Manager/Family Support Worker will provide home-based treatment, community-based outreach, comprehensive assessment, counseling, case coordination, and crisis intervention services for youths and families within the Department of Mental Health, Department of Children and Families, and Mass Health systems. The role requires a high level of flexibility, including evening and weekend availability, and a valid driver's license for travel.

Key Responsibilities:
  • Conduct intakes for new referrals to the program and educate caregivers on navigating systems of care.
  • Support caregivers in problem-solving and advocacy skills, locating and accessing resources, and developing strategies for self-care.
  • Provide in-the-moment modeling and coaching to increase parenting skills and sense of competency.
  • Collaborate with clinicians and other treatment team members to create strength-based client-focused assessments and treatment plans.
  • Facilitate case conferences and treatment meetings as needed.
  • Write progress notes, aftercare plans, service plans, and reviews as defined by the program contract.
  • Transport clients to court, appointments, and home visits as required.
Requirements:
  • Lived experience as a parent/caregiver of a youth with significant social, emotional, behavioral health, medical or trauma related needs and expertise in navigating child and family serving systems.
  • Bachelor's degree preferred.
  • Flexible schedule, including evening and weekend availability.
  • Excellent interpersonal, organizational, and writing skills.
  • Valid driver's license for travel.
