Job Overview

Hospice of the Western Reserve is a leading provider of end-of-life care, and we are seeking a skilled Clinical Telephone Triage Specialist to join our team.

This role involves providing high-quality patient care and support through telephone triage, working closely with our transdisciplinary team to coordinate care plans and ensure seamless transitions. Our ideal candidate will possess excellent communication skills, be able to work independently with minimal supervision, and demonstrate a strong commitment to delivering compassionate care.

Responsibilities
  • Analyze and assess patient needs through phone calls, identifying potential risks and developing effective interventions.
  • Collaborate with healthcare professionals to develop and implement comprehensive care plans, ensuring alignment with patient goals and preferences.
  • Communicate effectively with patients, families, and caregivers, providing education and support to promote self-management and empowerment.
  • Manage multiple priorities simultaneously, navigating complex systems and resolving conflicts in a timely and professional manner.
Requirements
  • A valid Registered Nurse (RN) license in the State of Ohio
  • At least 2-4 years of experience in clinical triage or call center settings
  • Excellent verbal and written communication skills, with ability to interact effectively with diverse populations
  • Demonstrated ability to work in a fast-paced environment, prioritizing tasks and managing time efficiently

Estimated Salary: $65,000 - $85,000 per year, depending on experience
