Job Summary

We're seeking a skilled Advanced Practice Clinician to join our team in Tolland County, CT. As a member of our interdisciplinary team, you'll work closely with patients, families, and healthcare providers to develop and implement personalized care plans.

Responsibilities
  • Conduct comprehensive assessments of patients and develop individualized care plans
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions
  • Prescribe medications and interventions to manage patient conditions
  • Participate in ongoing education and training to stay current on best practices and research
  • Contribute to quality improvement initiatives to enhance patient outcomes
Requirements
  • Completion of a Nurse Practitioner or Physician Assistant program
  • Current state RN/NP or PA license
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
  • Access to reliable transportation
Preferred Qualifications
  • 2 years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
  • Knowledge of long-term care, community resources, and cost-effective alternatives
  • Knowledge of CPT, ICD-10, and HCPC codes
  • Knowledge of clinical standards of care
