Key Responsibilities:
  • Comprehensive Patient Assessments: Conduct thorough initial assessments and ongoing care for patients in their homes, ensuring they receive the best possible care.
  • Collaborative Care Teams: Work closely with interdisciplinary teams, including PCPs, consultants, and health plan Medical Directors, to develop and implement patient care plans.
  • Primary Care and Support: Provide primary care and support to patients, including education on disease management and wellness strategies.
  • Quality Improvement: Participate in quality improvement initiatives to ensure the highest standards of care and promote clinical excellence.
Requirements:
  • Advanced Practice Clinician or Physician Assistant: Completion of an accredited program and current licensure in the state of Connecticut.
  • Certification: Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA).
  • Reliable Transportation: Access to reliable transportation and compliance with Optum's Motor Vehicle Safety policy.
