Position Overview:

At Yale New Haven Health, we are committed to fostering a culture where every team member embodies our Vision, supports our Mission, and upholds our Values. These core principles - integrity, patient-centered care, respect, accountability, and compassion - guide our daily actions as professionals.

The Advanced Practice Provider, encompassing both Physician Assistants and Nurse Practitioners, is a licensed healthcare professional who collaborates closely with attending physicians across various clinical environments. This role is pivotal in delivering exceptional diagnostic and therapeutic care, adhering to the highest standards of our organization as part of a multidisciplinary team. We prioritize a holistic approach to patient care, addressing the physical, psychosocial, spiritual, and functional needs of patients, their families, and significant others. The Advanced Practice Provider serves as a key figure in ensuring continuity of care, addressing and resolving issues within their scope of practice and specialty standards.

Key Responsibilities:

  • 1. Clinical Care: The Advanced Practice Provider conducts comprehensive assessments, triages, and manages therapeutic interventions for patients and their families throughout the continuum of care. This role is integral to the interdisciplinary team, providing optimal patient management from diagnosis to treatment. Responsibilities include obtaining medical histories, performing physical examinations, ordering and interpreting diagnostic tests, and formulating treatment plans. The Advanced Practice Provider may also perform procedures and prescribe medications in accordance with institutional guidelines and state regulations.
  • 2. Patient Education & Support: The Advanced Practice Provider educates patients and their families about care plans and available resources. Collaborating with supervising physicians, they provide information on treatment plans, potential side effects, and discharge instructions, while also offering psychosocial support in a compassionate and ethical manner.
  • 3. Information Management: Ensuring accurate documentation in compliance with hospital and regulatory standards is essential. The Advanced Practice Provider safeguards patient information across all formats from unauthorized access and disclosure.
  • 4. Quality Assurance: The Advanced Practice Provider is expected to uphold high standards of ethical behavior, professionalism, and commitment to patient care, ensuring compliance with healthcare regulations and standards.
  • 5. Professional Development: Continuous engagement in professional development activities is vital. The Advanced Practice Provider may serve as a clinical resource for new staff, mentor students, and participate in research and quality improvement initiatives.

Qualifications:
Education: Graduate from an accredited Nurse Practitioner or Physician Assistant program.
Experience: Clinical experience in the relevant specialty is preferred but not mandatory. All Advanced Practice Providers must be credentialed through the Medical Staff process.
Licensure: Active certification and licensure as required for the position, including ACLS/BLS certification and any additional credentials pertinent to the specialty.
