Senior Community Care Provider Opportunity

We are seeking a highly skilled Advanced Practice Clinician to join our Senior Community Care team in Connecticut. As a Senior Community Care Provider, you will play a critical role in delivering high-quality, patient-centered care to our members in a variety of settings.

Key Responsibilities:
  • Provide clinical coverage for primary APCs during periods of PTO or short/long-term absences
  • Manage members through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions
  • Conduct comprehensive, monthly/routine, and/or acute care visit assessments
  • Effectively manage acute and chronic medical and behavioral conditions
  • Ensure accurate and complete documentation, including ICD-10 conditions
  • Communicate and collaborate with the interdisciplinary care team
Requirements:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Active and unrestricted license in the state of Connecticut or ability to obtain by start date
  • Active Nurse Practitioner certification through a national board
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
  • Ability to gain a collaborative practice agreement, if applicable in your state
  • Ability to complete physical requirements, including lifting a 30-pound bag and performing physical assessments
  • Access to reliable transportation
Preferred Qualifications:
  • 1+ years of clinical experience in practice, preferably in a long-term care setting
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Proficient computer skills, including the ability to document medical information with written and electronic medical records
