Job Summary

The Health Services Coordinator plays a vital role in supporting the health and well-being of children and families across our programs. Reporting to the Head of Educational Excellence, this position is responsible for coordinating comprehensive health services, including medical and dental care, to ensure compliance with contract requirements and community care licensing standards.

Key Responsibilities
  • Collaborate with families to ensure completion of child health requirements, screenings, and assessments, and track health data to ensure compliance.
  • Work with parents to support medical and dental screenings, and ensure follow-up treatment is completed within established state and federal guidelines.
  • Monitor, guide, and support staff, providers, and families with children who have medical concerns across agency programs.
  • Review and evaluate health records to assess needs and plan special programs or activities to meet these needs.
  • Maintain accurate health records in hard copy and on the ChildPlus database, ensuring confidentiality and effective tracking.
  • Conduct and document follow-up on all referrals to ensure children's needs are met.
  • Work with families to ensure children's hearing and vision screenings are completed within 45 days of program entry.
  • Develop and implement individualized health and treatment plans as needed.
  • Provide consultation and technical assistance to parents, staff, and providers on health-related matters.
  • Share information with staff and relevant providers to ensure coordinated services that meet individual children's and families' needs.
  • Work with families and staff to support health-related training and education.
  • Participate in Multi-Disciplinary Team meetings and contribute to program evaluation and planning.
  • Assist in recruiting prospective families and facilitating the intake process for children with health concerns.
  • Develop and maintain partnerships with local agencies to support families in accessing health-related services.
  • Support the enrollment team by reviewing health documents and ensuring compliance.
  • Maintain accurate medication lists for early education centers.
  • Review, evaluate, and update health services reports, monitoring records for completeness and accuracy.
Qualifications and Education
  • Minimum of a Licensed Vocational Nurse (LVN) degree; Registered Nurse (RN) preferred.
  • Two years' experience working with children in the birth to five population preferred.
  • Strong communication and organizational skills, with ability to work effectively in a team environment.
  • Basic computer skills, including proficiency in Word, Excel, and software applications.
  • Experience working with diverse groups and populations, with a commitment to cultural sensitivity and awareness.
  • Bilingual in English and Spanish preferred.
  • Passionate about learning and committed to staying up-to-date on issues affecting children and families.
  • Fingerprint background clearance, or approved exemption, required.
  • Successful completion of pre-employment physical, PPD test/risk assessment, immunizations, and other required health clearances.
  • Class 'C' driver's license, proof of insurance, and reliable transportation required.
